Introduction

Patient Privacy is the MOST important target of our Company. We at Cognotrain believe that our data users privacy is the core value of all our services and platforms including our sites, app, onboarding site, software and hardware devices. CognoTrain (Including everyone working and providing service at CognoTrain) is/are committed to protecting the security, confidentiality, privacy of the account holders and the users (Patients) and any information provided in our platform. CognoTrain’s Privacy Policy describes how CognoTrain collects, uses, and shares your personal data.

 

At CognoTrain, We are protecting your Protected Health Information (PHI) and Personal Information including — such as your name and your patient’s name — is personal data, and also data that does not directly identify you, but that can reasonably be used to identify you. Any patient data including personal and medical information that are collected through the onboarding forms are stored in a HIPAA compliance platform.

 

Personal Data Used for Personalization

The main purpose of using Cognos Train is to provide the most personalized app and features that are unique to every user. The Personalized data are used to create unique games and interactive activities to help Alzheimer’s Dementia patients with their day-to -day life. The quality of the training and the care provided in CognoTrain depends on the accuracy of the personal data that the account holder( caregiver/family member) enters during the onboarding via the HIPAA compliance platform.  

De-identified Data

All the data provided to any of the CognoTrain platforms will be de-identified prior to aggregation and analysis. We at CognoTrain ONLY  may share the aggregated, anonymous and de-identified data which does not include ANY personal health information with the third party for the purpose of finding Novel drug discovery and medication, research publication and government funds.

Account Holder Privacy

The account holder is considered to be the family member or the caregiver who opened the account at CognoTrain on behalf of their patient and are required to enter their name, last name, address, email address, phone number , relationship with the patient/user and how many years they know the user. However, the purpose of  all this information is to have contact information about the account holder for the purpose of providing the most personalized platform for your loved one who needs help with their day-to-day life.  Here at CognoTrain, We do not SELL or share any of the account holders for any marketing purposes.

Personal Data Collected at CognoTrain

ALL THE PERSONAL DATA FROM YOU AND YOUR PATIENT COLLECTED IN COGNOTRAIN ONBOARDING FORMS ARE DESIGNED TO PROVIDE THE MOST PERSONALIZED AND CUSTOMIZED FEATURES IN THE APP FOR THESE PATIENTS USING THEIR OWN DATA TO HELP THEM WITH THEIR DAY-TO-DAY LIFE. 

1. Health Information:  Data relating to the health status of a patient, including data related to one’s physical or mental health or condition. Personal health data also includes data that can be used to make inferences about or detect the health status of an individual. All the health information entered in our platform will be aggregated and de-identified to be used for the research purpose to understand Alzheimer's disease and its progression for the purpose of drug discovery. WE AT COGNOTRAIN, DO NOT SELL ANY DE-IDENTIFIED HEALTH DATA WITH ANY THIRD PARTY.

 

1. Caregivers Information:  Information about the account holder including name, last name, address, email address, phone number , relationship with the patient/user and how many years they know the user. 

 

1. Payment Information: Information about the users subscription plan and payment, billing address and method of payment, such as bank details, credit, debit, or other payment card information, Transaction and subscription history.

 

1. Photo and Audio Uploads: This information includes uploading pictures and photos of the user, and the people that the user/patient is often interacting with and a short greetings audio recording. This information is for the purpose of creating interactive games to provide cognitive and memory training using patients data only.

 

1. Geographical information: Information about the patient residential location, address, the picture of the front entry and the locations that the user regularly visits. These information are for the purpose of providing the help for navigation for the users with geographical disorientation. The information provided will help to aggregate the app with the navigation features.

 

1. Medication History: Information about the medication, name of the medication, when and how it is supposed to be used and how it looks like and where the medication is located. These information are for the purpose of providing notification via the app to the user about when and who they need to consume the medication.

 

1. Milestones and life events: Information about patients' milestones and life achievements associated with its year. This information is for the purpose of creating interactive games and activities to provide cognitive and memory training using patients' data only.

 

1. Users Favorites: Information about patients' favorites in different categories. This information is for the purpose of creating interactive games to provide cognitive and memory training using patients data only.

CognoTrain believes  the patient's data privacy is the fundamental right of any individual. Here at CognoTrain, we DON'T  share any California consumer personal information or PHI data with third parties for any marketing  and advertising purposes.

California law allows California residents to ask companies with whom they have an established business relationship to provide certain information about the companies’ sharing of personal information with third parties for direct marketing purposes.
